We ask that any boat with more than three crew members at the party make a donation of $5.00/head; if you wish, you can pre-pay this with your entry fee. Please return this completed form together with $40.00 (or $35.00 for USSA members) made out to Holmes Hole Sailing Association, by Friday, September 1st, to:

RELEASE: In consideration of the acceptance of my application to participate in the George Moffett Race and with an understanding that there are risks inherent in sailboat racing, I do for myself, my personal representatives, family, heirs and assigns waive all claims against and release and discharge the Holmes Hole Sailing Association, its officers, members, volunteers and race committee personnel from any and all claims, demands liability, causes of action and damages of any nature whatsoever, as may accrue both now and in the future arising out of the participation of the above entry in any activity of the George Moffett Race. I represent and warrant that the above entry in this race will be outfitted, equipped, crewed and handled in such fashion as to be safe and seaworthy, will have all necessary equipment on board and will be competently crewed and skippered all in accordance with the applicable governing bodies. I acknowledge that the responsibility for operation of the above entry, including, but not limited to, the decision as to whether to race or to continue to race, rests solely, exclusively and finally with the owner and skipper.
